Adding a Graphic or Image to the top of each page on my Form
You can put an image at the top of each page using two different methods. You can insert a header/separation field item at the top of each page of the form and then use the "insert custom graphic" option.  Alternativly, if you wanted to show the same image on all pages you could do it outside of logiforms on the page that you add the integration code to. So, above where you put the form (using the generated integration code), you could insert a graphic. The later method assumes you are using the embedded integration option.

"Logiforms has allowed me to turn a somewhat tedious and often cumbersome
job into a simple and fast one. Most other hosted solutions do not allow
for the flexibility that Logiforms does. The features are well thought out
and provide a truly practical solution for any level of web developer.
Logiforms customer service and tech support has been second to none, and I have
never had a problem that the Logiforms staff hasn't dealt with in impressive time.
Logiforms sees their cutomers as paramount and has demonstrated this on several occasions.
I would highly recommend Logiforms to anyone who creates or manages one or more web sites."

"I use the autoresponse to send a slightly different "thank you" message to submissions, depending on their answers.
This is great, makes it really seem as though it is a personal response!"
